Event and Facilities Request
Below you will find the form required for requesting space in the facility and adding information to Calendar. Please review the following policies before completing the form:
- Form submission is required for events and building use.
- Forms must be submitted at least 6 weeks before the date of the event
- Forms must be submitted at least 6 weeks before the date of the event
- It is your responsibility to plan, gather, and completely think through details before completing your form(s). We have built the form with required fields to help with this process.
- If an event is recurring and takes place 1 time per month (or more), you only have to submit a form once per year to get it onto the calendar.
- Breaks of more than 2 months will trigger an additional form needing to be submitted.
- Breaks of more than 2 months will trigger an additional form needing to be submitted.
- Once the initial form has been approved, you will receive a confirmation email of that room or space will be reserved for you at the requested time.
